Morristown Vital Statistics maintains records of births, deaths, marriages, civil unions, and domestic partnerships that occur ONLY in the Town of Morristown. If you are looking for information regarding passports, divorce decrees, the surrogate or probate, they are located at the County offices on Court and Washington Street.  The County main phone number is (973)285-6000.

How do I get a birth certificate? Download the Vital Record form here: ENGLISH  // SPANISH

Birth Certificates are filed with the Registrar of Vital Statistics in the town where the birth occurred. Vital Statistics maintains records from 1905 to the present for births that occurred in Morristown. Certificates can only be obtained by the immediate family (Parent, sibling, or spouse)

  • You can apply for a newborn birth certificate approximately 15 days after their birth.
  • You do not need an appointment.
  • Certified copies are $25.00 each.
  • To request certified copies in person, come to the Vital Statistics window. The in-person process takes about 10 minutes. Download and fill in the form and bring a photo ID OR two (2) other forms of non-photo ID of the person making the request. Payment is Cash or Check ONLY.
  • To request certified copies by mail, You MUST include all the following: The form, your daytime phone number in case we have any questions, a photocopy of a photo ID OR two (2) other forms of non-photo ID of the person making the request . (An example is a phone bill or bank statement.) Payment is Check or Money Order ONLY.  Enclose a self-addressed stamped envelope and the check or money order payable to the Town of Morristown. Send the request to Morristown Vital Statistics, PO Box 914, Morristown, NJ 07963-0914.
  • Corrections made to all Birth Records incurred a processing fee of $50.00 and you need an appointment to file for a correction.

Death Certificates/Fetal Death Certificates are filed with the Registrar of Vital Statistics for all deaths that occurred within the Town of Morristown by a FUNERAL HOME. Records are maintained from 1905 to the present. Certificates can only be obtained by the immediate family (Parent, sibling, spouse, child of the decedent), legal representative or funeral home.

  • The cause of death is removed from the Certified Death Certificate. However, if a certification Form is completed, the cause will not be omitted.
  • You do not need an appointment.
  • Certified copies are $12.00 each.
  • To request certified copies in person, come to the Vital Statistics window. The in-person process takes about 10 minutes. Download and fill in the form and bring a photo ID OR two (2) other forms of non-photo ID of the person making the request. Payment is Cash or Check ONLY.

How do I file for a marriage license or civil union? Click here to download and print the form.

  • Apply for the marriage license in the New Jersey municipality where either applicant resides. The license is valid throughout New Jersey.
  • If neither applicant is a New Jersey resident, submit the application in the municipality where the marriage ceremony will be performed. In this case, the license is only valid in the issuing municipality.

For those who will be applying for their license in Morristown this is what you both must bring with you:

  • BOTH need current valid photo ID.
  • Proof that one partner lives in the Town of Morristown. (Driver’s license, bill, bonafide mail, etc.)
  • 1 witness over 18 years of age. 
  • $28.00 fee in Cash or Check ONLY.
  • We hold the license for 72 hours.

Divorce Decree

The Morristown Vital Statistics Department DOES NOT handle Divorce Decrees. You must contact the court in which you were divorced.
